You can provide an Alias, or just a first name.

Choose whether you want an audio or video custom.

Give your request an identifying title.

Include a start, middle, and end with the elements that are most exciting for you. Please include all critical details. Extensive dialogue is not necessary as I will not recite a script word for word, but feel free to give me example dialogue, phrases, etc.

I will reply with the final quotation and payment instructions after reading the form. Do not send payment before receiving your final total! It takes approximately 1-4 weeks for me to shoot, edit, and upload custom videos once I’ve received payment.
